Orchard Park Family Practice has made it possible to view their personal health information. The Patient Portal is an exciting and dynamic feature on Orchard Park Family Practice website. With the Patient Portal. OPFP patients can access and monitor their health information.

Log into the portal

The Patient Portal enables patients to

•    View key clinical information about their latest office visit
•    Monitor chronic health conditions
•    Request prescription renewals & receive notification when they are ready 
•    Receive email notifications when new information is placed in your chart
•    View your health history including test results
•    Schedule and/or cancel appointments
•    The ability to pay your bills online
•    Receive communications from providers with normal test results

Signing up for the Patient Portal is easy! Please stop at the front desk or call our office to receive an activation code that will enable you to login & create a user ID and password.
