New Patients

WE ARE ACCEPTING NEW PATIENTS!

WHAT TO EXPECT AND WHAT TO BRING

To establish care at Orchard Park Family Practice please call (716) 662-5357. At that time, you will be asked for some basic information including your insurance carrier, if there is any. This is so our billing department can verify participation.

Orchard Park Family Practice does not accept any Worker’s Compensation cases for new or existing patients.

Before your first visit we ask that you have your medical records sent from your previous office. We encourage you to activate your Patient Portal so that you have access to your chart. You must complete the new patient forms that are sent through a pre-check-link by email and/or text message. If you do not have a smart phone or email address, please print the new patient forms and bring the completed forms to your initial visit.

Please arrive 15 minutes early to your initial visit with a current list of your medications and a list of any other clinicians you are seeing like a Cardiologist, ENT etc.

Be sure to bring your insurance cards to each visit. We will take your picture to prevent insurance fraud at your Initial Visit.

Making an Appointment

  • Orchard Park Family Practice is unable to accommodate walk-in visits.
If you have a life-threatening illness or injury, call 911. We have appointments on the same day available in our Convenience Care. Please call as early in the morning as possible.

  • We require 24 hours advanced notice if you must reschedule or cancel an appointment.
Failure to do so will result in a No-Show fee. Patients who miss three appointments without the proper notification may be dismissed from the practice.

  • Our office will reach out 48-72 hours in advance to confirm scheduled appointments.
Please arrive promptly for your scheduled appointment. It will be necessary for you to reschedule if you are more than 15 minutes late.

  • Our providers' office hours vary and not all of them are available for evening or Saturday appointments.
Appointments for physicals annually are available at pre-determined times according to the provider you wish to see.
