Dr. Ruh has been a practicing Family Physician since 1993. He is in full time private practice and is an assistant Clinical Professor of Family Medicine at SUNY at Buffalo. Dr. Ruh is Board Certified in Family Practice and is a member of the American Academy of Family Physicians.

Dr. Ruh received his MD degree from University of Buffalo, an MBA from St. Bonaventure University, and a BA in English from Amherst College. He continues to see patients full-time at Orchard Park Family Practice and resides in Orchard Park, NY with his wife and 4 children.

Dr. DeGrave received his B.S. in Microbiology at the University of Notre Dame in 1982. After receiving his medical degree from the Chicago College of Osteopathic Medicine, he completed his residency in Family Practice at the University of Buffalo.

Dr. DeGrave is currently Board Certified in Family Practice and a member of the American Academy of Family Physicians. Along with his role at OPFP, Dr DeGrave is the Vice Chairman of the Catholic IPA, an 800-member, physician run network responsible for numerous quality of care initiatives taking place in Western NY. His hospital affiliation is Mercy Hospital of Buffalo.

Dr. Thomas graduated from Saint Bonaventure University with a BS in psychology and a minor in biology in 2003. In 2005, she received her Masters Degree from the University at Buffalo.

Dr. Thomas graduated from Lake Erie College of Osteopathic Medicine in 2009 and completed her residency training in Family Practice at Sisters of Charity Hospital in Buffalo, NY in 2012. She was Chief Resident from July 2010 to June 2011. Dr. Thomas is a member of the American Osteopathic Association and the American College of Osteopathic Family Physicians.

Dr. Prichard grew up in Erie, PA and attended Penn State University, where he received his B.S. in Biomedical Engineering in 2017 and his M.D. in 2023. He finished his residency training in family medicine at the University at Buffalo, where he served as Academic Chief Resident from 2024 to 2025.

He is a member of the American Academy of Family Physicians.

Amy Pohlman is a summa cum laude graduate of the Bachelor of Science/Master of Science degree program in Physician Assistant Studies from Daemen College. Her clinical career includes experience as a primary care provider for adult and pediatric patients in the areas of preventive medicine, chronic and acute illness management and women's health.

Ms. Pohlman began her career at Orchard Park Family Practice and continues after 6 years. Amy is licensed in the State of New York and she is certified by the National Commission on Certification of Physician Assistants.

Breanna has been a Physician Assistant at Orchard Park Family Practice since 2009, providing compassionate, comprehensive care to patients of all ages. She is dedicated to building strong patient relationships and promoting wellness through preventive and personalized medicine. Breanna has a special interest in women’s health, chronic disease management, and patient education. She values a team-based approach to care and strives to empower patients to take an active role in their health.

She is certified by the National Commission on Certification of Physician Assistants and remains committed to advancing the PA profession. Breanna serves as a board director of PAs for Women Empowerment and is a member of the New York State Society of Physician Assistants (NYSSPA) and the American Academy of Physician Assistants (AAPA). 

In her spare time, she enjoys spending time outdoors with her husband, 2 children and black lab.

Jill Kaczmarek FNP graduated Summa Cum Laude from D’Youville University with a Bachelor of Science in Nursing and subsequently Master of Science in Family Nurse Practitioner in 2000. She joins the practice with 25 years of experience in Pediatric & Adolescent Medicine. Jill was inducted into the Sigma Theta Tau Honor Society and is a past President of the WNY Chapter of NAPNAP Association.

Jill is a lifelong resident of Western New York. Outside the Office Jill is avidly into fitness both in gym and outside on the bike, traveling, fashion and the world of cosmetic injections.

Chelsea Denue, PA-C is joined Orchard Park Family Practice in April 2023. She is a 2007 graduate of SUNY Buffalo with a degree in Biological Sciences as well as Daemen College in 2010 with a Master of Science in Physician Assistant Studies.  She joins the practice with 13 years of experience in Emergency/Urgent Care Medicine as well as Pediatric/Adolescent Medicine. 

Chelsea is a lifelong WNY resident and grew up on Chautauqua Lake. Outside of the office, she enjoys spending time with her family and friends, getting back into playing the violin with her kids, staying active and traveling!
